You’ll need to download the questionnaire to your device in order to complete it. Look for the download link in upper right (click the 3 dots if you don’t see it) or just below the document preview.
Then answer the questions and return your questionnaire to me by email 24 hours before your session time. It’s fine to leave some of the questions blank if you don’t have an answer; just tell me what you can.
